1. Who we are and what we do
Coastal Stretch, operated by DC Wellness Group LLC, provides in-home wellness services focused primarily on assisted stretching and movement support. We will come to your home or another agreed-upon location to work with you directly. Our services are designed to support your comfort, mobility, and overall well-being. These are wellness services only — not medical treatment, physical therapy, or rehabilitation of any kind.
2. Assumption of risk
You understand that physical wellness activities, including assisted stretching, carry some level of inherent risk. These risks may include muscle soreness, temporary discomfort, or in rare cases, injury. You are voluntarily choosing to participate in these services knowing that these possibilities exist. DC Wellness Group LLC and its practitioners are not responsible for injury or discomfort that results from normal, good-faith participation in our services.
3. Wellness-only disclaimer (not medical care)
Coastal Stretch does not provide medical advice, diagnosis, or treatment of any kind. Our practitioners are wellness professionals, not licensed medical providers. The services you receive are intended to support your wellness goals and are not a substitute for professional medical care. If you have a medical condition or health concern, please consult your physician or medical provider before beginning services with us.
4. Pre-existing conditions disclosure
You agree to tell your practitioner about any known medical conditions or ailments, injuries, surgeries, or physical limitations before each session, such as joint replacements, osteoporosis, recent surgery, cardiovascular conditions, or chronic pain — but any relevant condition should be shared. This helps us work with you safely and effectively. DC Wellness Group LLC and its practitioners are not liable for the possible aggravation of any condition that was not disclosed prior to your session.
You confirm that you have disclosed all relevant health conditions, surgeries, and mobility limitations.
5. In-home service liability waiver
When services take place in your home or private residence, you agree to provide a reasonably safe space for your session. This means a clear area with adequate space and stable surfaces for movement and stretching, free from hazards, obstructions, furniture or other hazards. DC Wellness Group LLC and its practitioners are not responsible for hazards within your home — such as tripping risks, uneven flooring, or other conditions that are outside our control. We will always do our best to work safely in your environment.
You agree that DC Wellness Group LLC is not responsible for any hazards or conditions on my property, including slip-and-fall risks.
6. Cancellation and no-show policy
Note: Cancellations within 24 hours may result in forfeiture of the session fee.
You can cancel or reschedule your appointment in advance (more than 24 hours before your scheduled session time). Cancellations made with less than 24 hours' notice, or missed appointments without any notice (no-shows), may be charged the full session fee. Repeated cancellations or no-shows may result in a prepayment requirement or, if necessary, termination of services. We appreciate your respect for your practitioner's time, and we will always extend the same courtesy to you.
7. Photo and testimonial release
From time to time, we love to share the results of our work through photos, videos, and client testimonials. If you choose to opt in, you give DC Wellness Group LLC permission to use photos, videos, or written testimonials featuring you for marketing and promotional purposes, including on social media and our website. This permission is completely voluntary — your services will not be affected in any way if you choose to opt out. You may withdraw this permission at any time by notifying us in writing.
The signed agreement requires one choice:

□ I do NOT agree to the photo and testimonial release.
8. Release and limitation of liability
To the fullest extent permitted by law, you release DC Wellness Group LLC, its owner(s), agents, employees, independent contractors, and representatives from any claims, demands, or liability arising from your participation in our services — including claims based on negligence. This release does not apply to injuries caused by gross negligence or intentional misconduct. This agreement is binding on you, your heirs, and your legal representatives.
We take your safety seriously and will always act in good faith on your behalf.
9. Governing law and dispute resolution
This agreement is governed by the laws of the state of Florida. If a disagreement arises, both parties agree to first attempt resolution through good-faith conversation. If that is not successful, disputes will be submitted to mediation before any legal action can be pursued. We genuinely hope any concern can be resolved simply and respectfully.
10. Acknowledgment and signature
By signing, you confirm that you have read this entire document, that you understand what you are agreeing to, and that you are signing voluntarily. You are not under any pressure to sign, and you are welcome to ask questions before doing so.
The signed agreement records the client’s printed name, date of birth, emergency contact name and phone, client signature and date, and practitioner or representative signature and date.
